North Country Home Health & Hospice Agency (NCHHHA) offers home care, palliative care, and hospice services to the communities of Coös County, and Grafton County. Clinical teams are on call 24 hours/7 days per week.
Homecare is a service for those in need of medical treatment and/or assistance with the activities of daily living due to recovery after an acute illness, injury, disability, or exacerbation of a chronic illness. Generally, home care is appropriate when a person requires care that family and friends cannot easily or effectively provide on their own.
Palliative care is for people with a serious illness. It is available at any age and stage in your illness, and you can receive it along with curative treatment.
Hospice care is designed to support patients and families with the end of life transition by providing an interdisciplinary team approach, through quality care with pain symptom control, and emotional and spiritual support. Patients can continue to use and talk to their primary care provider, or call us directly. We have a staff of certified providers who can help assist, or can become one’s PCP, if you preferred.
