North Country Home Health & Hospice Agency (NCHHHA) is delighted to announce the promotion of Jessica Foster-Hebert, RN, CHHCM, CHCM, to Director of Clinical Services. In her new role, Jessica will provide oversight and leadership to the Home Health, Palliative, Hospice, and Long-Term Care business units, ensuring the highest quality of patient care and operational excellence.
With an impressive 13-year career in nursing, Jessica Foster-Hebert brings a wealth of experience and expertise to her new role. Her comprehensive knowledge spans various clinical specialties, including Home Health, Hospice, Oncology, Infusion Therapy, and Urology. Jessica’s commitment to patient-centered care and her extensive leadership experience make her an invaluable asset to the NCHHHA team.

Jessica has been a dedicated member of NCHHHA since 2019, serving as the Director of Home Health and Long-Term Care. During her tenure, she demonstrated exceptional clinical and operational acumen, leading to a survey deficiency-free year in 2020. Jessica also served as the lead RN, ensuring the highest standards of care for patients.
In her previous role, Jessica provided daily clinical and operational oversight for the Home Health and Long-Term Care units. She managed all clinical staff and excelled in case management, utilization review, and quality and financial outcomes. Jessica’s passion for building strong relationships within the healthcare community has contributed to increased referrals and improved patient outcomes. Additionally, she played a crucial role in orienting new hires, providing clinical competency training, and implementing process improvements.
Before joining NCHHHA, Jessica served as an Oncology and Infusion Therapy Nurse, providing direct care and overseeing oncology patient navigation while working closely with palliative care providers, She provided clinical support and training for all RNs and Medical Assistants and collaborated with providers to identify areas for improvement in clinical workflow processes.
In addition to her remarkable nursing career, Jessica also serves as a valued member of the Board of Directors at Androscoggin Valley Hospital, a testament to her leadership and dedication to healthcare excellence.
Jessica’s primary focus in her new role will be on increasing communication and improving transitions of care among referral sources. She has been working closely with North Country Healthcare Chief Nursing Officers and Chief Medical Officers, Case Management teams, and will begin collaborating with Primary Care Providers and the Mobile Integrated Healthcare (MIH) program to optimize patient care in the comfort of their homes.
